I think you'll be able to re-apply. It depends on how the deal was set up in the first place. Probably the domain is banned forever, though, would be my guess.
The Adsense account was in my name. I might try re-applying but I'm sure they will have banned the domain. I've just signed up with BidClix so I'll see what sort of returns I get from that.
Sorry to hear of your trouble Shane. If they have your social security number, they may have you banned personally. I have been hearing things about Quiqo AdSonar. They are in Beta but are supposedly comparable to AdSense.
I live outside the US so I can't remember whether I gave them my social security number or not. Probably not. I'll give Bidclix a bit of time. Maybe it will kick in for me. shane
Why not just purchase a new domain name and launch your site again and then reapply for an Adsense application agian?
they probably ban the webmaster as well as the website. they ban the tax id or ss #.

I don't know how lucrative it is yet because I just started, but it is the best AdSense alternative that I've seen so far. Customizability of the ad colors, font, layout, size is a big plus.
AdSonar sure has been taking a long time to roll out their product.
I guess they are trying to do it right. I didn't even think I would be accepted based on the text on their application page... but viola!
Share some experiences with them. I'm sure a lot of people are interested in learning how real the competition is for AdSense.
